Personal records missing

Personal information for about 4,500 children who have been clients of speech therapy and hearing programs offered at the University of Western Ontario is missing, the university says.
The missing records include names, addresses, phone numbers, genders, birth dates, doctor information, school and child-care information and, in 142 cases, OHIP numbers of clients who took part in two programs between 1999 and 2010.
UWO announced it was missing a USB memory stick containing client records.
